The 2026 Hawaii International Heart Failure Symposium is dedicated to addressing these pressing issues by equipping healthcare professionals with the latest knowledge, practical skills, and innovative strategies in cardiovascular medicine. The symposium will focus not only on advancements in diagnosis, therapeutic approaches, and cutting-edge research but also on promoting health equity, reducing disparities in care delivery, and fostering inclusivity in cardiovascular health Recognizing the vital role of healthcare providers' well-being, the event will include discussions and workshops aimed at addressing physician burnout, mental health support, and strategies to cultivate resilience in the demanding field of cardiology. By integrating clinical excellence with a commitment to equity and provider wellness, the symposium seeks to empower attendees to drive meaningful change and improve patient outcomes across all cardiology subspecialties, including medical cardiology, interventional cardiology, cardiothoracic surgery, electrophysiology, vascular surgery, and more. This event aims to inspire collaboration, innovation, and actionable solutions that advance cardiovascular healthcare while creating a more sustainable, inclusive, and equitable future for both patients and providers.

Objectives
  1. Describe the pathophysiology and progression of heart failure including distinctions among HFrEF, HFpEF, and decompensated heart failure.
  2. identify early clinical indicators of worsening heart failure and criteria that signal increased risk for decompensation or shock.
  3. Examine system level barriers that contribute to readmission, delayed GDMT optimization, poor adherence, and inequitable access to advanced HF care.
  4. Apply guideline based treatment algorithms and escalation pathways to common and complex HFshock case scenarios.
  5. Critically evaluate when to escalate to mechanical circulatory support using patient specific data, risk stratification, and organizational protocols.
  6. Explain the mechanisms, indications, and pharmacological profiles of contemporary HF medications including GDMT classes and therapies for specific conditions including hypertrophic and infiltrative cardiomyopathies.
