Overview

19.75 AMA PRA Category 1 Credits™

The Cedars-Sinai Transcranial Doppler Ultrasound for Comprehensive Stroke Care course includes lectures and hands-on sessions from experts in the field, and provides broad exposure to the theory, application, practice and interpretation of TCD studies. Topics covered include ultrasound physics, waveform analysis, vasospasm detection, intraoperative monitoring, screening in sickle cell anemia, PFO detection, vasomotor reactivity and evaluation for cerebral circulatory arrest.

Stroke encompasses many complex disorders that demand multiple imaging and diagnostic tests for optimal evaluation and treatment. TCD is used for standard-of-care evaluations (vasospasm, intracranial stenosis) as well as advanced applications (detecting PFO, intraoperative monitoring). TCD technology is portable, having versatile applications in clinic, the O.R., emergency room and inpatient settings. Comprehensive Stroke Center certification by The Joint Commission requires advanced capability and expertise in TCD for stroke evaluation.


Objectives
  1. Properly and appropriately establish SAH protocols
  2. Accurately interpret the waveforms for the diagnosis of cerebral vasospasm
  3. Identify and implement the criteria needed to produce an accurate Lindegaard Ratio
  4. Incorporate the variables uniquely found in the acute care ICU environment
  5. Identify intracranial disease processes and intervention to avoid stroke and TIA
  6. Increase proficiency of interpreting findings that are consistent with intracranial stenosis and understanding of collateral flow patterns
  7. Recognize when to include additional testing like embolic detection monitoring or Co2 challenge to improve diagnosis and interpretation of findings
  8. Follow the Stroke Prevention in Sickle Cell Disease (STOP) protocol to screen children with sickle cell disease
  9. Accurately interpret the findings consistent with increased stroke risk
  10. Identify the most appropriate time for patient follow up
  11. Discuss TCD bubble studies and the implications for PFO (persistent foramen ovale) screening due to its simplicity, noninvasive character, low cost and high feasibility
